How to register in person

If you would like to join the practice, you must bring proof of address (Utility Bill etc.) and your NHS (National Health Service) number with you when you come to register. If you have not completed the registration form, you will then be given one and Patient Questionnaire to complete. Once these forms have been received, you will be registered with the practice. If you are on medication, you will need a New Patient Check with the doctor.

If you are on regular medication, you will need to book a new patient review with the doctor.

Children 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

We would kindly request that applications are submitted by 6pm to enable the registration process to be completed that day.

Patient Services is a new secure system that will directly interact with your clinical medical record. This new system is already active and you can begin using it immediately after registration. With it, you will be able to:

  • Order Repeat Prescriptions
  • View Medical Records
  • Update Address, Mobile numbers and E-mail Contact Details

Please note that it is very important to inform the surgery of any changes to your contact details

Register at the surgery in person

Our Online Services are available for patients aged 16 and over. This allows you to order repeat prescriptions, view your medical records online and update your contact details. The service is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone.

Password and username details may be given to parents for under 13 year olds. Once they reach the age of 14 years old they will need to re-register for their own online access.

All you need is a unique Username and Password. To get these, simply call in to our reception, and our staff will be happy to provide them for you. You will need to provide the reception staff with photographic proof of identification such as a Passport or Driving Licence.

Once you have them, simply log-on via our website or by clicking here.

You will be asked to change your password to something more memorable for you.

The NHS app

If you are a patient at our practice, you can use the NHS App to access a range of NHS services on your smartphone or tablet.
It doesn’t replace existing services. You can still contact us in the usual ways.
Once you have verified your identity in the app, you will have easy, 24/7 access to a growing range of health services and information. You can:

  • order repeat prescriptions
  • access NHS 111 online
  • find NHS services
  • view your health records
  • book appointments
  • and much more…

We're serious about security

Your security is very important to us. We take a number of measures to help safeguard you while you are using SystmOnline.
